Each professional provides the same objective, that of maintaining the maximum quality of life for the patient and family, for as long as possible.
This is achieved through an initial integral assessment of the situation, leading to the establishment of a personalised care treatment plan.
Continuous follow-ups and support are made through telephone contacts and further visits to the out-patient clinic.
When the patient is no longer able to attend the out-patient clinic, the home care teams will assure continuation of the care and support in the patient’s home.

Support to the family is extended through out the bereavement period.

Telephone support
To complement the out-patient programme, direct telephone support service is available until 19.30h. every day of the year. In this way continuous follow up of the patient's and family´s needs are assured, giving the opportunity to consult on any query.
