Fundación CUdeca Cudeca

Our care

An advanced illness requires comprehensive and personalised care, which we offer through different programmes and a team specialised in palliative care.

At Cudeca we care for people with advanced illnesses and their families, offering palliative care that is comprehensive, professional and compassionate. Through our specialised teams, we provide care at home and in our inpatient units, accompanying every stage with a commitment to relieving suffering and improving quality of life.

Home palliative care for adults

En Cudeca, we offer palliative care directly in the patient's home, combining specialised medical care with psychological and social support for both patients and their families. Our goal is for everyone to receive the care they need without having to travel, in a safe and familiar environment.

Our teams consist of doctors and nurses specialised in palliative careand are complemented by psychologists and social workers. This allows us to offer care that is fully adapted to each patient's individual situation.

The home visits take place from Monday to Friday, mainly in the mornings, and always in coordination with the Primary Care teams of the Public Health Service, which guarantees a comprehensive and continuous attention.

Initial assessment

When we begin care, we perform an all-inclusive assessment which allows us to:

  • Assess the symptoms caused by the condition.
  • Evaluate the patient's functional status.
  • Conduct a nutritional follow-up.
  • Evaluating the emotional state.
  • Review the social situation, family network, employment status, and the need for orthoprosthetic equipment or home adaptations.
  • Understanding the wishes and preferences of the patient and their family regarding treatments and care.

Based on this assessment, the team develops an individualised care planwhich adapts to the patient's needs and wishes, and can be adjusted as they change over time.

Continuous monitoring

Care does not end with the initial visit. Our medical and psychosocial team provides constant follow-up through phone calls, home visits, and consultations at the Cudeca Palliative Care Centreensuring that the patient and their family receive continuous and personalised support.
