Patients may be referred to Cudeca Hospice by their hospital consultant, family doctor or other health professional. Referrals normally are for purposes of pain and symptom control, also for rehabilitation following radiotherapy, chemotherapy, surgery and other palliative interventions, psychological and social support. Referrals may also be for specialist respite or terminal care.
A patient and/or the family member can contact Cudeca directly and ask for help, in which case we request that all medical reports are presented at the first appointment.
To request a first appointment, either visit the Hospice directly or make contact by telephone. Speak to the Medical Secretary who will arrange the first appointment.
If you need advice about referring yourself or someone else to one of our services, or simply require more information, please do not hesitate to contact us. Telephone 952 564 910 or use the email contact form
